HCD International (HCDI) has joined forces with a local Health Department to strengthen Long-term Services and Supports (LTSS) for elderly and chronically ill residents. This partnership is focused on connecting individuals with the care and resources they need to live safely and independently at home.
“Every person deserves access to high-quality, personalized care that allows them to thrive in the setting that suits them best,” says Drummond. “Our mission is to identify the right mix of services and make those connections happen quickly and effectively.”
As part of the initiative, Community Health Nurses will engage seniors, disabled persons and their families to complete applications assessing various LTSS and related needs. These incredible nurses bring deep compassion and unwavering dedication to the work—ready to serve and make an immediate impact.
