Social Determinants Indicator e-Tool
The HealthCare Dynamics International (HCDI) is delighted to share the ‘Caring for Your Health’ Social Determinants Indicator e-Tool (CFYH e-Tool). HCDI developed this tool in response to clinician concerns regarding the social factors affecting their patient population, the impact of these factors on clinical outcomes, and the subsequent negative adjustment that can result from value-based payment model. These social factors may also affect the patient’s ability to self-manage and adhere to their shared decisions. This brief questionnaire is patient facing and provides real-time, up-to-date information to you, the provider, while creating an opportunity for shared decision-making to identify patient preferences to improve the quality of their care. The data collected through this tool includes REaL data, demographics, social determinants of health and customized data. HCDI provides free training and technical assistance to effectively integrate this tool into your already established workflow processes.
The ‘Caring for Your Health’ Social Determinants Indicator Tool is both a patient level and population health management tool that:
- Provides real-time opportunities to identify socio-economic factors that can affect the patient’s clinical outcomes
- Assists in patient risk stratification
- Allows for documentation of the patient case complexity
- Aids in development of early interventions
- Promotes health equity
Leveraging the information obtained from CFYH Tool: ICD-10 Coding
ICD-10 Coding has greatly expanded the number and specificity of codes available to completely describe a patient encounter. Among these new codes are the Z codes related to family and social circumstances impacting health status. Data on social determinants of health collected through Z codes may be used in population health management. Z code data on social determinants of health can supplement traditional clinical patient data to improve patient management. A practice identifying a defined process for collecting and aggregating data on social determinants of health is an important step toward patient population management and for practice transformation.
